For licensing a Non-Medical Home Care Agency in Virginia, your NPI should be Type-2 (business) and linked to your agency name. However, obtaining your license requires more than just an NPI.
To get licensed, you’ll need to submit several key documents. First, provide basic business and identity details, including the owner’s driver’s license, resume, Employer Identification Number (EIN), and, if you’re an LLC, your operating agreement. Next, demonstrate your agency’s structure and governance, which means listing the names and titles of your governing body and submitting meeting minutes that document your startup decisions.
On the financial side, you’ll need to show proof of a physical office space (such as a lease or rental agreement), submit a business plan, a budget outlining projected revenue and expenses, and a bank statement as proof of funds. From an operational perspective, you must submit an emergency preparedness plan, the administrator’s job description, their resume, and a complete set of policies and procedures that detail how your agency will comply with Virginia regulations.
Finally, your agency must have proper insurance coverage, which includes at least $1 million in general liability insurance, a third-party crime insurance policy or fidelity bond of at least $50,000, and proof that the insurance is under your agency’s name and address.
